To enroll in a Bachelor’s Essay or Individual Enrollment Type Course you must contact the desired Academic Department. Students do not have access to Application for Individual Enrollment Forms and must ask for the applicable form from the Academic Department.

Types of Application for Individual Enrollment Forms:

• Application for Individual Enrollment
• Application for Individual Enrollment – Express I or Express II
• Application for Individual Enrollment – Bachelor’s Essay
• Application for Individual Enrollment – After the Deadline

Checklist for Students:

1. Contact the academic department about regulations governing the intended Individual Enrollment. 
2. Begin a dialogue and have a commitment from the faculty supervisor at least one semester in advance. 
3. Ensure that this form is filled out completely.
4. Attach a detailed project description or syllabus, and include the means by which the faculty supervisor will grade the student’s work. 
5. Attach a copy of your Degree Works degree audit. 
6. Submit application to your Academic Department.
